Introduction

Supported living for adults with complex needs

A unique supported living concept, The Fallowfield Project is made up of three houses that come together to form their own community. We support male and female adults with complex needs, including autism, learning disabilities, or a mental health condition.

We support people to live as independently as possible, but some may not fully understand their needs. In some cases, they may be subject to conditions under the Mental Health Act, 1983.

Located in the heart of Fallowfield, we provide 24-hour support across three spacious homes. Working closely with the landlord, we ensure that everyone has their needs met, both through the environment and the support we provide. Our non-judgmental, inclusive approach values every individual, treating them with respect and dignity. We’re committed to improving their quality of life, helping them to live as independently as possible within a nurturing, positive community. We hear every voice, meet their needs, and empower them every step of the way.

Who we are

Here at The Fallowfield Project, we believe in building a warm and welcoming community where every person can thrive and live life to the fullest. Everyday we are welcomed into the homes of the people we support, helping with:

  • household management
  • safety
  • budgeting
  • developing and maintaining social skills
  • making, managing and attending appointments.

All our support is tailored to the individual and delivered in the way that suits them best.  

Our team is driven by compassion and respect. We provide person-centred support that encourages people to live independently. Embracing each person’s unique needs and perspectives is at the heart of what we do. Our non-judgmental, inclusive space ensures everyone is empowered to shape their life and choices.

Our supported living approach

Our supported living model is designed to develop or maintain independent living skills, helping to build self-confidence and self-worth. Tenants are actively involved in shaping their daily lives.

We actively support individuals in shaping their daily lives. Beyond just maintaining the tenancy agreement, we focus on promoting independence through daily living skills, healthy eating, and food preparation. We also help with household responsibilities and support planning outings and holidays, making sure there’s always something exciting to look forward to. And that’s just the beginning! There’s so much more we do to help everyone live their best life.

Our homes and facilities

Consisting three spacious houses, each within a short walk of the other, The Fallowfield Project creates a strong sense of community.

Each house promotes independent living with shared communal spaces, including kitchens, lounges, bathrooms, and large gardens. These spaces encourage respectful co-living while helping everyone make the most of their daily living skills, with staff support available when needed.

Tenants have their own lockable bedrooms that they can personalise, offering a sense of privacy and ownership.

What is supported living?

Supported Living, sometimes known as assisted living, is designed to help individuals live as independently as possible. Tenants receive personalised 24-hour support and care to help them thrive in a space they call their home.

What are the tenancy arrangements at The Fallowfield Project?

At The Fallowfield Project, each person has their own tenancy agreement. Tenants have their own lockable room that they are encouraged to personalise giving them privacy and a sense of ownership.

Who is supported living suitable for?

The Fallowfield Project supported living service empowers adults with diverse needs. We provide person-centred support for individuals on the autism spectrum, with learning disabilities, mental health conditions, alongside physical health issues.

Does The Fallowfield Project accept out of area referrals?

Yes, we can accept out of area referrals.

How long are tenancies at The Fallowfield Project?

There is no end date or set period for our tenancy agreement. Tenancies are tailored to individual needs and can be short-term, longer-term or lifetime. Care act assessments are carried out every 12-18 months by the commissioning teams to see if the placement still meets the needs of the tenant.
