Empowering families with revolutionary care

Every human counts

Our leading human rights-based approach has been recognised at the CQC’s Annual Equality Conference and is the cornerstone of our organisation. We place individuals at the centre of everything we do, providing activities that promote choice and environments that encourage independence. Upon admission, each resident is assigned a key worker as part of their wider team. All key workers work with each individual to create a personalised plan around their preferences and needs.

Part of the community

We enable our residents to lead the most independent life possible. We adopt an inclusive approach to delivering care and believe that the wider community plays an essential role in supporting the health and wellbeing of our residents. Our homes and hospitals are well established, our residents and tenants can integrate with their local communities without fear of stigmatisation or exclusion. From theatre trips to gardening groups, we identify and engage with community-based events endeavouring to enrich the lives of the individuals and family members we support.

Support for families

We care for the wellbeing of every single person that steps through our doors including partners, friends, and family. We know the invaluable role you play in providing comfort and aiding recovery, and are committed to supporting you in this crucial role. As a service we provide comprehensive guidance on our accommodation and protocols, running family forums and social events such as Christmas parties and barbecues, encouraging a wider sense of community. Here at Equilibrium Healthcare, we involve relatives in care planning wherever it is appropriate.

Services that connect

We are committed to developing new technology to help improve the lives of our residents and to keep family members connected. We are proud to launch several new initiatives throughout the year, including our state-of-the-art virtual consultation room, and enabling video conferencing across all our locations – giving relatives direct access to the practitioners in their family member’s care team. We break barriers often felt within the healthcare community, service users, and family members to forge strong connections no matter the distance.

CQC report, Fallowfield Project, October 2020.

“The philosophy of the service was to focus on the individual rather than a diagnosis or offending history and to recognise and celebrate people’s unique values, personalities and personal histories. Everyone was considered an equal partner in their personal support journey. Tenants were supported to express their views in a way that was personalised to the individual.”
