Requires Improvement

Introduction

Caring for adults with complex needs

At Oakland House Nursing Home, we foster community, compassion, and care, creating a home where everyone feels valued and respected.

With a focus on tailored support, we have two floors dedicated to caring for individuals with enduring mental health needs and co-occurring conditions.

Our separate ground floor unit provides specialised care for people living with dementia, supporting those with complex needs that impact their health, mobility, and cognitive functioning.

By prioritising person-centred care, safety, respect, and continuous improvement, we’ve built an environment where people are supported to thrive.

More than a nursing home

Oakland House is more than just a nursing care facility. We’re a family dedicated to helping people stay independent and feel their best, whilst facilitating their nursing needs.

Specialising in the care of people with enduring mental and physical health needs, as well as cognitive decline including dementia, we understand that each person’s journey is unique. That’s why we think outside the box to ensure everyone gets the right support.

Our management team includes a registered manager, deputy manager, and clinical lead. They work closely with nurses, health practitioners, the OT team, and support workers on each unit.

Together, we create a compassionate, tailored approach where people feel safe, supported, and able to thrive.

Person-centred care

Person-centred care is at the heart of everything we do.

Working together, we create tailored care plans that reflect what matters most, ensuring choice, promoting independence, and guiding daily support.

Regular reviews ensures care plans remain responsive and meaningful. As needs change, we adapt plans, assess risks, and update documents to reflect the right level of support.

This ensures every person gets the care they need, when they need it, in a way that matters to them.
