We accept referrals 24 hours a day, 7 days a week. You can make a referral using our simple online referral form or by contacting the service you require directly.
Make your referral to Equilibrium Healthcare via our Referral Portal, or directly to the service you require.
If suitable, an assessment will be arranged by the service manager, and undertaken within 5 working days.
An assessment pack will be provided within 5 working days, including service outcomes and funding requirements.
Feedback will be provided to the referrer on whether our service can meet the needs of the service user.
An admission date will be agreed, following funding approval from the commissioning organisation.
